What is Support Coordination?

Focus’ Support Coordinators will assist you to build the skills you need to understand, implement, and use your plan. They will work with you to ensure a variety of supports are used to increase your capacity to maintain relationships, manage service delivery tasks, live more independently, and be included in your community.

A Support Coordinator with Focus will support you to understand and implement the funded supports in your plan. We will support you to achieve your goals build your independence and live your life the way you want to. Focus’ Support Coordinators will assist you to liaise with providers about the services they provide and assist with your everyday life. We will ensure service agreements and bookings are completed.
